It is a request form, not ‘enter and receive immediate access’ form. You’ll need to wait for Shopify to review it and get back to you with a decision.
We recently received this information. It takes 1-2 days and you should be good to go. 
We requested 3 apps: 2 development apps and 1 staging app.
Shopify responded and made it clear that we have to request each app individually.
This makes zero sense to me. I will also need one in the future for our production app. Now I have to send a separate request for every app that I need.
My opinion is that they should open this API for development apps. When submitting for review, they should ask developers to fill out this form if their app requires this scope.
