Production POS extension - customer can't load - worked previously

I have a customer using a POS extension I created for them. They used it all this past weekend and it worked as expected. Today they see “Loading…” in the POS tile, followed by “App failed to load”. No changes were made to the app.

I tried doing a fresh deploy with no impact.

This is in production and I would expect a stable deployment for this production version of the app. Am I missing something in my set-up?

If you re-launch the app or re-add the tile extension does this resolve the issue?
